Social Workers, MSW's, are state licensed individuals who work with patients to assess the full range of their needs and help them access the services and resources available to them to improve their quality of life. 

Examples of Social Work services:
  • Provides an assessment of a patient's needs during the transition from a medical facility to home to assure the patient and their family have the support they need to maintain the patient's highest level of independence.
  • Assists families with applications, phone calls, and obtaining information about all community resources available for support.
  • Provides education and counseling to the patient and family about their condition.
